The church is located along the route of the ancient Centurelle-Montesecco sheep track and is dedicated to St. Mary of Graces (Stefania Di Primio). Its construction date is unknown, but it has existed since the 16th century. It is a rural church with a bell gable and a simple façade featuring a stone portal surmounted by a large window. The two low square windows are typical of this type of church: they allowed shepherds to attend religious services while remaining outside to watch over the flock. On either side of the two windows, the names Sulmona and Civita (probably Chieti) are inscribed, likely serving as road signs. The interior has a single nave with two side altars, dedicated to St. Peter (TU ES/PETRUS/ET SUPER/HANC/PETR/AM 1638) and St. Paul (PREDICATOR/VERITIATIS/IN UNIVERSO/MUNDO/ F.ED.EPS/A.N.D./1627).
On the back wall, above the central altar, there is an important fresco depicting the Madonna with the Child Jesus and Saint Pantaleon, protector of doctors, whose author and date are unknown. The church also preserves some plaster statues (once colored and now with very rare inscriptions) depicting the saints, Joseph and Joachim (Pater Virginis) on the left, and on the right, most likely the prophets, Kings of Israel, David and his son Solomon.”
